I know this is an old topic, but I have a 16gb sandisk USB flash drive, my windows (vista) machine always tell me that format did not complete successfully.

Have tried the HP formatting tool, same result.

What to do?

Did you do it exactly this way?  Exactly?

I thought you just right click on the drive (if it exists) and use the format option? Or click on format if computer detected formatting required?

That's how the HP format tool works too.

That's how I formatted my old USB flash drives on my computer in the past . I already have backed up all the files on my drive so I don't care if the stuff on my flash drive gets erased.

My flash drive in question, I had been using without problem for a while, but lately it's been sitting unused and now when I attempt to use it,  it sort of hangs up now and then, forcing me to have to unplug the device and replug it in to get it working again. So I thought I'd just reformat the device. But it's giving me the error.....

I have a failed flash drive that prompts for a format and then says it couldn't complete. I know it's failed because it's literally reporting itself as a totally different device. (it was a SanDisk Cruzer, now it's reporting itself a "SanDisk Pen USB".
I was trying to dereference Null Pointers before it was cool.

    1.  Since you don't care about the "data", you are free to "aggressively" test the flash drive media for read/write errors.

These programs may be used to test the media:
         a.  Check Flash 1.10
         b.  h2testw 1.4
         c.  USB Flash Tools
         d.  USB Flash Drive Tester 1.14

    2.  Fake/Counterfeit USB Flash Drives:
         a.  CAVEAT EMPTOR, BEWARE of Counterfeit USB Flash Drives
         b.  I want to believe: Fake USB Flash Drive

I did use checkflash program you have listed, and it tells me there's lots of errors, even on the simple one round check. This was a flash drive I got off of Amazon one day and for a 16gb size, I'm hoping I don't need to throw it out if it's still usable 'cuz 16 GB is still plenty of space on it.

Thanks for the links dahlarbear, I'm going to check some of my old drives, see if they really are failed or just purtending :P
