Table of contents in Microsoft Word

I'm working in a very long Word document that has about a 3 page table of contents at the beginning.  After edits are made to the document, I'm trying to update the entire table (Right Click/ Update Field/ Update entire table).  When this action is taken, it duplicates some of the lines in the table (some of the bookmarks are listed twice.. some with different pages than the original), and I'm not sure how to make this stop happening. 

I didn't create the document, so I don't know if something funny might've been done.. or where the bookmarks are being pulled from.

Does anyone know anything about this, and how I might be able to keep the duplicate lines from my document?
