My two USB ports from motherboard and three from PCI card are not working and I'm in desperate need of assistance. So far I have:
Checked in BIOS and indicates USB is ENABLED and supporting high speed.
Device Manager says everything working properly, shows no problems. Checked updates just in case, all the most recent are installed.
Went into Power Management and unchecked box " allow computer to turn off devices to save power"
I then disconnected pc form wall outlet for an hour in case pc was holding settings
Powered up and ports still not working.
Is there anything else I can check or do, I'm wide open to suggestions

if you remove the pci usb card do the ones on the mother board work??
start there, if they don't work..can you disable them and just try the pci card ones???

Win XP Home edition with SP3
I just had eye surgery and will have to get assistance in removing pci card but I disabled all ports and then Enabled them one at a time, tried them individually. I also did everything previously mentioned in SafeMode also, no luck
I tired both printer and digital camera in each port too, but to no avail
That's where I sit now, Dead in The Water ???

Humor me...
disabling is not good enough...REMOVE the pci card, then boot up,goto control panel, system, hardware, device manager and select the plus sign next to universal serial bus controllers, right click each controller and select uninstall...re-boot and let them be configured, check the 2 usb ports...report results...

When I got this thing apart, then realized there is no pci usb card . my Intel D865perl mobo has 4 internal header connections that must be controlling the front usb ports
USB Ports 4 from Back Panel, 4 from Internal Header - USB 2.0 (from Intel)
Yes, the USB controllers are detected in device manager.
Uninstalled, re-booted, still no operating USB ports

A 4 port PCI/USB card with firewire also will run you about 15 to 30 dollars depending on where you are...
Installation with help from a friend should take no longer than 15 minutes tops.
Although not the most glamarous solution probably the least painful...

What kind of USB devices are you trying to connect?

I have a Lexmark 8350 printer that I only use once a month or so ,just to keep the ink moving. Rarely use it.
And a Fuji digital camera. This whole thing came to light when I went to upload a couple of pics to the computer.

What happens if you remove the camera from the device manager and then reconnect it?

I can't find the camera in device manager unless it is covered by " plug and play software enumerator" which is active. This is not new hardware, both printer and camera about two years old and have been working fine. My son sometimes used a little portable drive that contains his resume etc in the usb port too which never gave a problem.

I'd delete all ghost entries related to USB devices.

Alright, and what about any other USB devices that can be removed?
By the way, do you recall what happened to your PC before his happened? Did you install a new driver, a Windows update or anything else?

There are no other USB devices on the pc. It has been some time since I used a USB port for anything. I check for updates every other day and update when applicable. The computer works great otherwise, quite fast, no nasties on board etc, regular scans ???

Try the following;
Delete HID and Composite USB (Human Interface Devices)
Delete USB Root Hubs (Universal Host Controllers)
Delete USB Host Controllers (Universal Host Controllers)
Then restart.
Or go with what Patio suggests for a quick workaround. Chances are your USB ports might jump back to live after one update or another in the near (or far) future.

I don't think uninstalling SP 3 will help unless you uninstall SP2 as well. When you download USB drivers for a mainboard, the manufacturer often says that you should either load these drivers or update to SP2 which would lead me to believe that the support for a lot of USB controllers is in SP2 and not SP3 or SP1.
Anyhow, it wouldn't be very hard to reinstall. You can get the downloader here (http://www.microsoft.com/downloads/details.aspx?FamilyId=68C48DAD-BC34-40BE-8D85-6BB4F56F5110&displaylang=en)
I'd keep that as a last resort and if you're in a really experimental mood. You might damage your Windows installation if something goes wrong.
Try completely uninstalling all USB related devices first.
By the way, could you try booting from a USB device to see if the ports and controller are still functioning on a hardware level? Or hooking up a USB keyboard and entering the BIOS from there. You might need to enable 'USB Legacy support' for USB hardware to work during POST.
IF you can't boot from a USB device, try going into safe mode and hook up a USB device there to see what happens.
By the way, are you absolutely 100% positive that your BIOS is properly configured when it comes to USB devices?

In BIOS it says........USB boot.....ENABLED
Then in advanced, i go to USB Configuration and it says:
High Speed USB........ENABLED
Legacy USB Support...ENABLED
USB 2.0 Legacy Support..... Full-Speed

Might not be there if you do not have a USB mouse or keyboard (or any other input device) so you can skip that.
BIOS seems fine indeed.
Restart your computer to let the changes take effect, Windows will most likely need a minute or so to reinstall the drivers.
If that didn't work, try booting from a USB device or enter the BIOS using a USB keyboard so that we can test the controller and USB ports without Windows interfering.

I don't have a USB keyboard.
I uninstalled the five Universal Host Controllers and uninstalled the five USB Root hubs and rebooted. The controllers and the root hubs reinstalled themselves. I did exactly that at least twice yesterday with same results as just now.
The only devices I have that connect via USB are my camera and printer.
I haven't tried removing SP3 yet, can I remove that and install SP2 to see what happens ? I read somewhere recently that SP3 has been causing various problems for some folks, for apparently no rhyme or reason. I don't know ??? ??? ???

First try booting into safe mode (F8 before Windows loads) and using a USB device from there.
it'd be really handy dandy if you could find a USB key or mouse/keyboard.
Something less complex than a camera or a printer...

Confirm that your USB ports and controllers work at all by the aforementioned method(s).
If they do not, you will have to invest in the solution Patio offered.
If they do, you could look into uninstalling SP3 and SP2. However, I think it is less time consuming and more safe to simply reinstall Windows after having made the proper back-ups...
Lets first confirm the status of your controller O.K?

You're not following me.
Test the controller and ports by booting from a USB device or entering the BIOS using a USB keyboard.

Hello Again:
I installed a PCI USB card, but there is something that is keeping it non-operational, although it shows up in Device Manager ????
But, my USB ports from motherboard are now working, but only with my sons thump drive, not my camera or printer.So I completely uninstalled printer and camera from system, even removed all entries I could find in the Registry. I have not re-installed yet.
Is there something I should do before reinstalling camera & printer software, or should I install and hope for the best ?? Thanks

So far, so Good :) Printer is once again working, and camera is uploading pics. Both are working from USB motherboard ports, which was the initial problem way back at first Post.
The USB card is just sitting in the pc doing nothing and can continue doing nothing as along as the motherboard ports work.
I have no idea why the USB ports started working, did nothing more or less than I did everyday for the last several years ???
