Title: MS Word question

I have a link on a website that goes directly to a word document.  The document is a form that users can print out, fill in, then send back.  I have been trying to find a way to create a space where users can type in the information for the form, so they could download it, fill it in electronically, then email it.  Now, when I try to type information into the form, items on the same line move as if it were a regular document.  I know I have created documents for surveys that have a space that will allow the user to enter in his own information, however I cannot remember how I did it.  How/Can I create an "answer box" without it affecting the rest of the words on the same line?  Will I be able to make the changes without recreating the entire document?  

I am using MS Word 2000.
