At first my computer was working in top notch; however I had several issues with " Black/White/Gray/ etc etc color screens; I presume it was overheating since No virus, malware was present. With overheating as my first guess; I start to clean my CPU heat sink [ Fan ] . while using a Q-tip on the heat sink dust wasn't present. Therefore I had no issues with Color Screens. Everything was working fine for several months.

However a month ago I had no problem with my USB ports; then I had a " Unknown Device " Install. With that problem arise; I did several forum/solution search.

1 - Start in Safe Mood - Control Panel > Device Manager > Universal S bus > Unknown Device > Right Click Uninstall > Restart [ This didn't solve the issue; at desktop re-installing that "Unknown Device" was still present ] Next I decide to uninstall ALL USB > Windows reinstall. Problem no longer exist.

However two days ago; I was on my computer watching anime on youtube then my screen with Black. Power was present in my tower and my monitor was working. Since I couldn't do anything; I hold my power button for a few seconds [ 3 ] it shut down safely. Pushing the button once more; the light came on/ off / on / off / on.

Finally it decide to stay constant; however passing boot screen was an issue. I try a few times and the same results with power / off and on / + Cannot pass boot screen. So i was thinking could it be a corrupt software; let me system restore to an earlier point. While i was in the process of system restore; it frozen once more. I waited for 15 mins to see if it was move; but nothing. Restart once more to system restore to see I didn't any my "Points". Without any options left system factory was decided. I back up my data [ So it wasn't an issue ]

After system factory, everything was fine. My drivers + USB were working like brand new. However yesterday [ Oct 11 2011 ]; I had that problem once more " Unknown Device " Installing.

Software such as FireFox/AVG/CCleaner/Gooogle Chrome/Speccy/Open Office/Unknown Device ID 7.00/ were my only downloads.

Using Unknown Device

* If the full DeviceList notepad is needed, simply ask. *
* Using unknown device ID 7.00 I cannot find any problems listed [ No red lines of any device ]

________

Update [ Oct 15 ]

Methods I did

1 - Uninstall every USB > Reboot > Windows Install > Problem once more
2 - " " [ Safe Mood ] > Reboot > Problem once more

3 - Update Nvidia Drivers / USB Drives / Windows SP1> SP2
3 - Malware scan; No Malware / Any form of Virus /
4 - System Factory > Problem came up after a week days [ As i mention above ]
5 - DriverScan > All my USB [ Green Check ]
6 - Uninstall Display Adapter [ Nvidia Geforce 9100] > Problem exist

Try reinstalling the drivers for your motherboard chipset in addition to troubleshooting each individual USB device-I had the same problem on an MCP61-GeForce 7025 chipset motherboard.
Basically one day my Standard OpenHCD controller drivers were not working-but My Standard Enhanced PCI to USB Host Controllers were. Several "Unknown Devices" appeared.

Just to be safe; may I ask where exactly I need to uninstall the [ Motherboard chip set ] Better to be safe than sorry. I have to make sure it's the correct way of doing this.

In addition I will Device Man > USB Controller > Uninstall every sub division. Reboot and see what happens ?

You don't need to uninstall-you can just download the setup exe and do a repair install.
Yes you can uninstall every subdivision of the USB controller section. The drivers will be automatically reinstalled at reboot.

USB has Malfunctioned? Have you tried with different USB Mass Storage devices? Have you tried plugging it into the back of your computer? It could very well be that one of your USB devices is drawing too much power-try plugging all of your USB devices out of your computer, leaving only the USB drive in question plugged in.

I tired back and front. However no form of USB are in use. I'm using a normal port for my mouse and keyboard.  What I was thinking if it's possible to temporary disable that one Bad USB [ Unknown Device ] If that is possible.

Okay... do other USB devices work? Can you try the USB drive on another computer?
It is possible to disable it in device manager:

Universal Series Bus Controllers
-standard enhanced pci to usb host controller-
-standard enhanced pci to usb host controller-
-standard openHCD usb host controller-
-standard openHCD usb host controller-
- Unknown Device -
- USB Root Hub-
- USB Root Hub-
-USB Root Hub-

If this helps

Most likely it is your USB drive that is broken-have you tried to plug it into another computer?

Sorry for the late reply. I had my finish my midterms. Regardless; I believe it's a hardware problem as several people mention in the forums. However I was thinking if it's possible to disable that one bad USB and not let windows auto install that single driver once more.

USB Flash Drive work perfectly on other computers. I believe it's the actual physical hardware of the USB.

So we have narrowed it down to that one malfunctioning port. Plug the USB drive into the malfunctioning port, remember which one it is in device manager, and then plug it out, then disable the malfunctioning port. Disable it, not uninstall, so that Windows won't prompt you to reinstall the port's drivers.
