Hi fellows
can someone explain to us how the grey fields were created in the following URL form?

It's not a "URL form", it's a Word file.  This may help: http://www.viddler.com/explore/MargaretLoader/videos/66/.  For more, Google on creating forms with Word 2007 or similar phrase for your version of Word.  Or, use Word's Help feature and search on forms.
