Title: How to design a ballot for Windows Front Page

I would like to place a ballot within a web page designed by Front Page 2000. It would be a yes or no question and voters would be able to see the result as soon as their vote is cast. Result would be displayed by a line graph showing percentage yes and percentage no. I have seen this format used on news sites such as CNN. Can anyone tell me how to design such a ballot or refer me to a source that contains the info? Thanks.
