Are the keyboard & mouse both ps2?
Could you have interchanged the plugs?
They don't work in each other's sockets.

If you have pluged or unpluged the kbd or mouse while system is powered up, you have most likely burnt a fuse on the mother bd. You may be able to find and replace or jumper it. Usually near the ps2 ports. No fuse...then expect a burnt trace in controller chip.

When you formatted your hard drive you remove the ability of your computer to recognize your usb mouse.  Do you have a standard mouse to put into your p/s 2 connection? Try that and see if it will work OK?
