Use your router's port forwarding feature.  Do you know how?  You may also want to consider using a Dynamic DNS service, unless you have a static IP.

Sure.  When you're connected to the router, your network's external IP address won't be shown by the game, but that's the address your friend will need.  Have a read of the networking FAQ to understand more (link is in my signature).

you could always just go to http://www.whatismyip.com/ to find your external ip address.

Lots of good stuff to read here:
          www.portforward.com/default.htm

"Our Routers  section offers detailed walkthroughs on how to setup port forwarding. These how to guides make it easy to setup your router or firewall for any application you may need."

          www.portforward.com/help/portforwarding.htm
          Explains what port forwarding is.

          www.portforward.com/routers.htm
Is your router on this list?

"These are the devices that we have written support documents for. These documents provide step-by-step guides on how to setup port forwarding."

          I hope that helps.
