Help with TOC and INDEX

Win 10, Office 2010, large document with a long TOC.  Must make  an index with Hyperlinks back and  forth between the TOC and the index.  How do I accomplish this.

Is the final document going to be:
• A web page in HTML format.
• A digital document in PDF.
• A interactive media presentation.

You can make a hyperlink document in MS Word using the native word format. However, the target user has to use a word reader to view it.
