How to fix infection? Antivirus unsuccessful.

Hello, I am having a problem with squares appearing in the tiltle bar of IE (all versions). When it occurs there is freezing and overall slow performance. I have tried using CCleaner, Mcafee antivirus, and Nod32 Antivirus. Here is a copy of the Hijack log for anyone who would like to help. I have also attached a screenshot.

I'm getting the same thing on Youtube for the last few days. It must be something new on their site. I just now discovered that it only shows up when you sign in to YouTube. If you don't sign in, there are no boxes.
Correction on that. The boxes only appear when you do a search.
