Need help removing UACINIT virus

My computer will not boot into normal mode.  I can go into safemode.  I followed some tips from other posts and was able to get Malwarebytes to update and run by renaming the executable.  It comes back and says it will remove C:\windows\system32\uacinit.dll on reboot.  However it does not. Any help on removing this virus is appreciated.  Below is my Hijackthis log.

Thanks for the help so far.  I followed what instructions I was able to do.  Was not able to boot into normal mode.  I believe the virus is still keeping me from doing some this on your list.  Here is my latest HIJackThis log.  As you can see I still have the uacinit virus.

I downloaded SAS.  However I get a message that "The system administrator has set policies to prevent this installation."

You might have to run them in safe mode or possibly rename the files.
