When it comes down to it, making the processing pages, and whatever else it requires is quite difficult!
There is an alternative:
Please download the web application located here:
http://sourceforge.net/projects/phpformgen/

Although Dreamweaver is a very cool program, it will not do all the PHP form sending for you.
You have to make it yourself, and that, my friend, is very difficult....
