    I'm working on a survey to track equipment in various departments.  I have it set up already so staff would identify which items they have, but I also want to know how many of each item (which would likely need to be an open text field).  Is there a way to do that in the same question?  The way its set up currently is a question that asks them to check the box next to each item they have.  Ideally I could add an additional field after each item for them to enter how many they have, but if thats not possible, it would be nice to have the same list in the next question with blank text fields to enter the quantity.  Problem is it seems like I can only have one open text field per question so if there's not a method I haven't thought of, I'd need to list each item as a 'yes/no' do you have it' question followed by a blank text field where they'd enter the quantity.
