Once you have defeated all the other enemies surrounding Shub-Niggurath, you will want to make your way up towards the portal. Do not go into the portal but instead drop down off the side and make your way to the second portal. Watch to the right as the floating ball enters into Shub-Niggurath's back right as he enters the back enter the portal and you will win the game.
